Guiding Life’s Next Chapter.
Compassionate, personalized guidance for senior living placement, downsizing, and relocation, so your family can make an important decision with clarity and confidence.

A personal guide, not a directory.
When a parent or loved one needs more support, families are often asked to make big decisions quickly, while juggling work, emotions, and a long list of unfamiliar options.
Everly Senior Transitions was created to lighten that load. Kim Mazzetti works with your family personally, listening first, then helping you explore senior living options and coordinate the practical details of downsizing and moving.
Think of Everly as a concierge for one of life’s most meaningful transitions: thoughtful, organized, and focused entirely on your family.
More than a move… a brighter tomorrow.

Frequently asked questions
What does a consultation involve?
It is a relaxed conversation, by phone or in person, about what your family is facing. Kim will ask about your loved one’s preferences, the kind of support that may be needed, location, budget, and timing, then explain what options and next steps could look like. There is no obligation.
Do families pay for your services?
It depends on the service. Before any work begins, we explain in writing which services carry a fee, which do not, and whether a senior living community compensates Everly. You will always know where things stand.
Will you only suggest communities that pay you?
No. Your loved one’s needs, preferences, and budget come first. You are always free to choose any community, including communities that have no arrangement with Everly.
Are you a medical or care provider?
No. Everly does not diagnose conditions, perform clinical assessments, or determine whether someone qualifies for admission. Those decisions rest with qualified professionals and the receiving community. We help you understand your options and coordinate the practical steps.
What is the difference between assisted living, memory care, and board and care?
Assisted living communities offer help with daily activities in a larger setting. Memory care provides a secure environment and specialized programming for people living with dementia. Board and care homes are smaller licensed residential homes with a more family style setting. Our Resources page explains each in more detail.
I live out of the area. Can you still help?
Yes. Many families coordinate a loved one’s move from a distance. Kim can keep you informed by phone, email, or video call throughout the process.
You don’t have to figure this out alone.
A first conversation is simply a chance to share what is happening and hear what your options might look like. There is no obligation.
